Patients should be aware that fees will be charged for services not covered by the NHS e.g. private certificates, reports, insurance claims etc. Medical reports and examinations for insurance are usually paid for by the insurer.

Fees will be charged for HGV & PSV licences, Elderly drivers, fitness to travel certificates, sports fitness certificates, private sick notes, passport applications, etc.

All requests need to be put in writing and these requests will be dealt with within 2 weeks.
